As people hit their deductibles and OOP maxes over the course of the year they tend to use more healthcare. I.e. if you need a knee replacement at some point in the not too distant future and you've already hit your deductible for the year you might as well do it for free this year rather than wait until all that resets next year. This is also why hospital revenues tend to be highest in Q4...
